29 CFR 1910.23(b)(9): (9) Ladders were not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury; On or about March 4, 2016, employees were exposed to a fall of approximately 8 feet. The employees were removing solar panels from an existing residential home located in Moreland Hills, Ohio, 44022. The employees were using an adjustable Gorilla extension ladder to access the roof. The ladder had not been inspected as frequently as necessary to identify visible damage.
