OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in-that employee(s) were exposed to crushing hazards: (a) Head Frame Area - Jib Crane/Hoist No. 1: Below the hook lifting device (in house fabricated lifting hook and spreader bar) used to lift head frame components and other material did not have the rated load capacity and identification nameplate marked/affixed to the lifting device. This exposed the employee(s) to a crushing hazard. (b) Head Frame Area - Jib Crane/Hoist No. 2: Below the hook lifting device (in house fabricated lifting hook and spreader bar) used to lift head frame components and other material did not have the rated load capacity and identification nameplate marked/affixed to the lifting device. This exposed the employee(s) to a crushing hazard.
