29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position. (a)The Teledyne Pines bender was not properly locked out while conducting maintenance on the bending arm of the machine. A padlock was not attached to the lockout hasp and safety tag that make up the energy isolating device needed to to isolate the energy to the machine.
