29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): On or about February 16, 2017, and times prior thereto, the employer did not protect employee(s) from cave-in hazards when working in a greater than 5 foot in depth trench while stick welding on a 10 inch steel gas pipe, by the use of excavation protective support and shield systems.
