29 CFR 1910.28(b)(6)(i): The employer did not ensure each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminate the hazard. NAPCO PLATING LINE: On or about and at times prior to February 14, 2017, the employer did not provide guard rails or other restraint system to protect employees working on the platforms around the NAPCO plating tanks. Employees perform job-related tasks while on these platforms multiple times each day.
