29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) Honda Transmission Manufacturing of America Inc. 6964 State Route 235N Russells Point, OH, 43348: On or about January 13, 2017 operator employees were exposed to an amputation injury in that the Mesh Line 9 station 3, was not locked out prior to clearing a jam. The employer did not ensure that energy control applications were utilized and steps to control the pneumatic energy sources were implemented prior to unjamming the machine; As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, clearing a jam.
