29 CFR 1910.147(c)(4)(i): The employer did not develop, document and utilize procedures to control potentially hazardous energy when employees were engaged in any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: a) In the Warehouse; on or about January 6, 2017- The employer did not develop or implement any energy control procedures for the baler equipment. Employees reached limbs inside the operating hydraulic baler to remove cardboard stuck from the edges.
