29 CFR 1926.502(d)(6)(i): A snap-hook not of the design type was engaged directly to webbing, rope or wire rope: (a)Hultgren Construction, LLC at 132-136 S. Phillips Ave., Sioux Falls, SD 57104: On or about and at times prior to December 2, 2016, the controlling and exposing employer did not ensure each temporary employee using a personal fall arrest system while working on a scaffolding system, was not wrapping the lanyard around the structural member of the scaffolding, and connecting back into the webbing of the lanyard. The lanyard and snap-hook were not specifically designed for the purpose of tying back into the lanyard. In the event of a fall, the snap-hook gate could be exposed to forces in excess of the design specifications causing it to break or deform. This condition exposed an employee to a fall hazard of approximately 18 feet.
