29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation. (a)Hultgren Construction, LLC at 132-136 S. Phillips Ave., Sioux Falls, SD 57104: On or about and at times prior to December 2, 2016, the controlling and exposing employer did not ensure the narrow base scaffolding system utilized base plates. The narrow base scaffolding fabricated frames were placed directly on the floor without the use of base plates or casters located in the Copper Lounge and Skelly's. This condition exposed employees to falls up to 12 feet.
