29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury: (a)Hultgren Construction, LLC at 132-136 S. Phillips Ave., Sioux Falls, SD 57104: On or about and at times prior to December 2, 2016, the controlling and exposing employer did not ensure the employees were provided eye protection when projectile hazards created by pneumatic nail guns, hammer drills and sledge hammers were present. The condition exposed employees to the hazard of eye injuries.
