29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a)Hultgren Construction, LLC at 132-136 S. Phillips Ave., Sioux Falls, SD 57104: On or about and at times prior to December 2, 2016, the controlling and exposing employer did not ensure that employees and temporary employees wore protective helmets when exposed to overhead hazards such as, but not limited to, falling tools, materials, and debris. This condition exposed employees to struck-by hazards.
