29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Production Floor, Line #2: On, or about, November 15, 2016, an employee was injured when he attempted to clean the scrap cutter blade on the 540 Ravioli machine. He lifted an interlocked cover and reached in before the blade stopped rotating. The machine was not locked out during the cleaning process.
