29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: On or about November 21, 2016 employer did not ensure that the forklift in need of repair was taken out of service. The forklift was being operated while the backup alarm and the horn were not operative. This condition exposed employees to struck-by hazards.
