29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. On or about November 14, 2016, the employer did not ensure that employees utilized energy control procedures to control hazardous energy prior to cleaning the Line 4 Potato Hopper. An employee received a partial finger amputation while cleaning the Line 4 Potato Hopper in that the equipment was not properly deenergized.
