29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Unit 300 Phenol cake wash filter area. On October 30, 2016 one chemical operator trainee was changing filter media on filter 3259S when he was supposed to be changing the media on filter 3259 and an authorized person had not place a lock on the lock out box.
