29 CFR 1926.416(a)(3): Before work began the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. Nor did the employer post and maintain proper warning signs and advise employee(s) of the power circuit as required: a) Building I2, Mezzanine Level; Switchgear S&T - On or about October 19, 2016 and times prior thereto, substation S side of the 480 volt switchgear was energized and the employer failed to assess the substation T side of the switchgear to determine if exposed bus bars were energized. Employees working on and near the substation T side of the switchgear were exposed to electrocution hazards. b) Building I2, Mezzanine Level; Switchgear S&T - On or about October 19, 2016 and times prior thereto, the employer failed to post and maintain proper warning signs and instruct each employee in the recognition and avoidance of unsafe conditions while working on and near the partially complete and energized switchgear exposing the employees to electrocution hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
