Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and electrical parts were deenergized: a) Building I2, Mezzanine Level; Switchgear S&T - On or about October 19, 2016 and times prior thereto, the employer did not ensure that a qualified person tested, by use of a test instrument, that the load side of the electrical circuit in cabinet #4 was verified as being in a deenergized state. Employees were exposed to electrocution hazards while working on and near exposed bus bars of the 480 volt energized switchgear. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
