Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1926.502(d)(6)(v): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. a) At Pump House Location: On or about August 22, 2016 and at times prior; the employer allowed his employees to set his snaphook from his Personal Fall Arrest System on the holes in the coffer damn walls which would not allow the snaphooks to lock and could easily detach from the wall exposing the employees to a fall hazard of up to 18 feet to the ground below. The holes cut into the coffer dam wall were not large enough for the employee to run their snaphook and to tie off back to itself.
