29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: The employer is failing to prevent employee exposure to electric hazards. On or about 08/15/2016, at the Third Street Bridge Viaduct project in St. Louis, Missouri, an employee pulled a 25 foot rebar from an expansion joint on the bridge. The rebar contacted a 4 kV phase to phase/ 2.4 kV phase to ground power line that was located approximately 9 feet from the bridge. The employee was electrocuted as a result of this condition.
