29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about August 23, 2016 and times prior at the tub wash machine; not lockout out during cleaning: The employer failed to ensure procedures were developed and utilized to lock out the tub wash machine when employees are cleaning the inside and the machine is not properly locked out which exposes them to the hazards associated with inadvertent release of energy such as electric and water.
