29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 08/01/2016 - Maintenance cage, a MSC drill press model #00085420, serial #433206 was being operated with an unguarded rotating chuck. b) On or about 08/01/2016 - Maintenance cage, a Vectrax milling and drilling machine model #00492108, serial #434151 was being operated with an unguarded rotating chuck. c) On or about 08/01/2016 - Maintenance cage, a Trump vertical milling machine model #K3V1F, serial #K3V104 was being operated with an unguarded rotating chuck. d) On or about 08/01/2016 - Maintenance cage, a 6-inch belt sander model #09514506, serial #036022 was being operated without a guard for the ingoing nip point. e) On or about 08/01/2016 - Maintenance cage, a MSC horizontal band saw model #473260, serial #200407 was being operated with an unguarded rotating blade.
