Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) crushing injuries from the superstructure's swing radius of the 235B caterpillar excavator not being barricaded while swinging to the operators right side. The swinging placed the counterweight within inches of the 6 inch wacker water pump during the dumping of the bucket of dirt removed from the north west end of pier # 2 cofferdam. Employees were working in this area placing a cover over the exhaust pipe of the water pump. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..." (a) Install a barricade warning system around the superstructure's swing radius of the caterpillar excavator.
