Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the crane operator was exposed to the hazards of operating a crane that was not actively maintained: (a)Sun Sand and Gravel Inc., at 1.9 Miles S.E. of Highway 24 on Tigiwon Rd., Minturn, CO 81645: On August 8, 2005 the employer did not ensure an active preventive maintenance program, including detailed records was established and implemented for the safe operation of the crane. This condition exposed the employee to hazards such as but not limited to: inadequate lubrication, excess dirt, grease, or foreign matter, cracks and leaks, excessive wear, improper torque, cracked or broken structures and welds, improper alignment and insulation of wire rope, and damaged, frayed, or broken wire rope strands. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure preventive maintenance is performed on the crane as directed by ANSI B30.5-1968, The American Society of Mechanical Engineers - An American National Standard - Safety Standard for Crawler, Locomotive and Truck Trains Section 5- 2.3.1. Manitowoc Crane Care has a Grove Maintenance Inspection Manual prepared for use when performing required maintenance and inspection. The manual also serves as an inspection reference for those items to be checked on a mileage basis.
