Text For Citation: 01 Item/Group: 001 Hazard: LACKTRAIN
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the crane operator was not properly qualified to operate the truck crane: (a)Sun Sand and Gravel Inc., at 1.9 Miles S.E. of Highway 24 on Tigiwon Rd., Minturn, CO 81645: On August 8, 2005 the employer did not ensure the operator of the Grove RT635C truck crane was qualified to operate the crane in that the operator did not take a practical operating examination. This condition exposed the employee to the hazards of operating a crane for which he was not properly qualified. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the operator is properly trained and qualified as directed by ANSI B30.5-1968, The American Society of Mechanical Engineers - An American National Standard - Safety Standard for Crawler, Locomotive and Truck Trains Section 5-3.1.2. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
