Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electric shock: On or about July 26, 1999, an employee using the hand operated elevator located at the workplace was exposed to fall hazards and crushing hazards created from the elevator not being inspected and maintained in accordance with the American Society of Mechanical Engineers ASME A17.1 - 1996. The hand elevator was not inspected and/or maintained as follows: a)Elevator inspection intervals exceed six months. b)The counterweight was not secured by at least two (2) tie rods passing through holes in the sections or equivalents. c)The elevator did not have a car safety device attached to the underside of the car frame, capable of stopping and sustaining the car and rated load. d)The elevator entrance was not equipped with a guardrail or barrier to prevent employees from entering the car loading area. Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1)Inspect the hand elevator at intervals not longer than six (6) months as required by ASME 17.1, Rule 1010.3; and 2)Secure the counterweight by at least two (2) tie rods passing through holes in the sections and having lock nuts at each end, secured by cotter pins as required by ASME 17.1, Rule 606.1; and 3)Provide the elevator with car safety, attached to the underside of the car frame, capable of being kept closed as required by ASME 17.1, Rule 602.1; and 4)Provide a self closing or manually operated horizontally slid or swinging hoistway door capable of being kept closed as required by ASME A17.1, Rules 600.5a and 600.5b 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
