29 CFR 1910.119(f)(1)(i)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address normal operations. On/about June 7, 2016 and at times prior thereto at the Tyson Foods facility in Hope, AR the employer operating procedures that directs employees in adding refrigerant to the ammonia refrigeration systems were not consistent with the process safety information when: a) SOP-6.2 instructed operators to operate vlaves that are not included on the referenced P&ID b) Pressure indicators specified in the procedure do not appear on the reference P&ID c) Procedure refers to obsolete drawing
