Text For Citation: 01 Item/Group: 001 Hazard: BURNS
IC 22-8-1.1 Section 2: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Disa Number One - An employee was exposed to molten metal burns when he kicked the valve of the autopour box with his foot when the box was full of molten metal. "Among other methods one feasible and acceptable abatement method to correct this hazard is to repair or replace the valve and train employees not to use their foot to kick the valve." (b)Area Seventy One - Employees were exposed to the hazard of burns from molten metal in that ladles were being transported with molten metal up to the top of the ladle which could result in spills." Among other methods one feasible and acceptable abatement method to correct this hazard is to develop, implement and enforce a procedure for filling the ladle." (c) Disa Number Two and Three - employees were exposed to the hazard of burns from molten metal in that the pour platform that was approximately eight inches wide in some areas did not provide enough room to move away from any spill. "Among other methods one feasible and acceptable abatement method to correct this correct this hazard is to increase the size of the pour platform." (d) Disa Number One - employees were exposed to the hazard of burns from molten metal in that the pour platform that was approximately twelve inches wide in some areas where employees poured molten metal did not provide room to move away from any spill." Among other methods, one feasible and acceptable abatement method to correct this hazard, would be to increase the size of the pour platform." (e) Area Seventy One Melt - employees were exposed to the hazard of burns from molten metal in that no visual warning device was provided for forklifts that transported molten metal around where other employees walk and work." Among other methods one feasible and acceptable abatement method to correct this hazard is to install a visual warning device on forklifts that transport molten metal." (f) Inducto Therm and Ajax Pour Platform - employees were exposed to the hazard, of burns from molten metal in that no audible alarm was used to inform or warn employees when the furnaces were dumped." Among other methods, one feasible and acceptable abatement method to correct this hazard is to install an alarm system that sounds when furnaces and dumped." (g) Cupola - employees were exposed to burns from molten metal in that the alarm that was used to warn employees when the bottom doors are to be dropped was not operable." Among other methods one feasible and acceptable abatement to correct this hazard is to repair or replace the alarm system." (h) Area 71 - employees were exposed to molten metal burns in that there was no preventative maintenance program for the refractory of the molten metal furnaces." Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain records, of heats so as to better predict when furnaces could burn through." (i) Area Seventy-One - there was no preventative maintenance program for the refractory of the molten metal ladles that can result in burn throughs.
