DBA Freshway Foods, located at 601 North Stolle Avenue, Sidney, OH 45365: On or about May 11, 2016, the employer did not ensure that the rotating horizontal shafts of the Line 1 product conveyor were enclosed in a stationary casing. Employees were exposed to caught-by and laceration hazards.
