29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a) 50 Community Street, Wellsville PA - On or about April 8, 2016, employees repairing and/or replacing purlins in a metal roof system were not protected from falling 24 feet to the concrete floor below. While removing concrete form panels used as a walking/working surface, an employee fell through a rusted hole in the existing metal roof and sustained a fatal injury. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
