Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed: (a) Location - 800 South St., Waltham, MA: On or about 6/16/99, an employee was exposed to and received serious crushing injuries when a radio tower, 300 feet high, weighing 14,000 lbs., was raised off its cement pad approximately 20 inches with two 10 ton hydraulic jacks to replace the damaged base insulator while an employee was cleaning underneath. Among other methods, feasible and acceptable to correct this hazard include, but are not limited to the following: a. Utilize a long handle brush so as bit to have any body parts under the load. b. Follow manufacturer's recommendation (safety information) to block raised loads.
