Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple traumatic injuries, fractures, crushing injuries and/or death by a fallen steel "I" beam: Worksite: Employees on 10/4/92 were standing on a steel "I" beam, used as a guide in sheet piling operations, while a sheet was being driven into the ground. The welds on the beam failed causing the beam and the employees to be thrown to the ground, with the beam landing on top of the employees. One feasible method of abatement, among others would be to prohibit employees from standing on the guide beam during the actual driving of the sheet into the ground. (This item was abated during the inspection.)
