Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing and lacerating hazards as methods of communication was not established prior to structural steel beams being handled by a gantry crane: (a)High Steel Structures, Incorporated, Plant #4 yard - The ground crew employees, involved with the operation of the MI-JACK Travelift 750D crane, were not provided information regarding the proper methods of communication with the crane operator on or about October 1, 2004 ABATEMENT NOTE: One useful and feasible abatement method is the conspicuously post hand signals as illustrated in Figure 10, Standard Hand Signals for Controlling Overhead and Gantry Cranes, of ASME B-30.2-2001 (Safety Standard for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks and Slings). Additionally, as per ASME B-30.2-2001, Section 2- 3.3: Signals, 2-3.3.2 Special Signals, (a) through (c), which state: Special operations may require additions to or modifications of standard signals. Special signals shall be agreed upon and understood by the signalperson and the operator. Special signals shall not conflict with standard signals. Additionally, the employer shall assure the MI-JACK Travelift Crane is operated in accordance with the manufacturer's Travelift Crane 650C/750D Operator's Manual, 39700153-R6. VERIFICATION REQUIRED
