29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. On or about 13 January 2016, and at times prior thereto, electrical outlets located in walls as completed installations did not have the faceplate, exposing employees to an electrical hazard of approximately 120 volts. A) Conference Room: On the east wall, the two-plug outlet was missing the faceplate. The outlet was energized at the time of the inspection. B) Small Break Room: On the south wall, the two-plug outlet was missing the faceplate. The outlet was energized at the time of the inspection. Also, the west and north walls both had two-plug outlets that had damage to the faceplates, but not to the extent the terminals or wires were visible.
