29 CFR 1910.179(n)(4)(i): The upper limit switch of each hoist was not tried out under no load, at the beginning of each operator's shift. On or about 13 January 2016 and at times prior thereto, employees operating overhead cranes did not test the upper limit switch under no load at the beginning of shift, exposing employees to the hazard of being struck by falling loads.
