29 CFR 1910.179(n)(2)(ii): The load was not attached to the load block hook by means of slings or other approved devices. On or about 13 January 2016 and at times prior thereto, a spreader bar of unknown origin with no manufacturer noted nor load test documentation, was being used by employees to lift steel plates up to approximately 18,377 pounds in the layout area, exposing employees to the hazard of being struck by falling equipment and loads.
