29 CFR 1910.146(d)(4)(i): Under the permit-required confined space program required by paragraph (c)(4) of this section, the employer did not ensure that testing and monitoring equipment needed to comply with paragraph (d)(5) of this section was maintained properly. On or about 13 January 2016, and at times prior thereto, workers welding inside either vessels or units were provided with personal gas monitors that had not been calibrated through bump testing prior to each day's use, exposing the workers to an asphyxiation hazard.
