29 CFR 1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation. On or about 13 January 2016 and at times prior thereto, an employee was operating a metal plasma cutter without shaded eye protection, exposing the employee to the hazard of light radiation.
