-DELETED- 29 CFR 1926.452(w)(1): Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections shall be secured. (a) Merkling & Associates, Inc., @ 0026 Aster Court, Eagle, Colorado 81631. On or before July 6, 2015, as part of injury/accident prevention responsibilities, the prime/exposing interior finish carpentry employer did not ensure that the mobile fabricated frame scaffold was braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. All brace connections were not secured. The scaffold was missing the diagonal brace at the base frame and the cross brace or other suitable systems at the working platform to make the scaffold plumb, level and squared. On July 6, 2015 a carpenter working from this mobile scaffold, who was installing wood beams on the ceiling, fell over the left side of the working platform approximately 6 feet to the floor. The carpenter suffered a fatal head injury.
