29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about November 19, 2015, at 3051 Kumho Parkway, Macon, Georgia, in the curing building: An employee installing the Electronic Monorail System wiring was exposed to a fall hazard of approximately 16-feet when he worked from cartwalk 2 with unguarded floor openings and was not tied off which resulted in a fall which caused his death.
