29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: The following flexible cords were observed with evidence of damage and were not removed from service: a. Employees had taped an extension cord where damaged being used for the dust collection system near the SCMI table saw. Tape covering damage was observed in two locations on the cord. b. The employees had taped a damaged area for a flexible cord attached to a fan being used in the paint room.
