29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. East Dock, Overhead Door No. 6 - On or about November 17, 2015, a maintenance employee was required to lockout a dock platform using two maintenance locks (props), a storage lock (prop), and lockout hardware (bolts and nuts). The lockout hardware (bolts and nuts) used by the employee on the maintenance locks (props) were not the appropriate size to secure the dock platform in the service position (vertical). The employer did not ensure that the employee(s) were provided with the appropriate lockout hardware (bolts and nuts) and that the lockout hardware was made available in the workplace to secure the dock platform in the service position (vertical). Employees were thereby exposed to struck-by hazards.
