29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. (a) U.S. Department of Agriculture; Food Service Inspection Service at Tyson Foods, Inc. 11224 Lankford Hwy. Temperanceville, VA 23442: On or about October 23, 2015 at 10:45 PM, a Food Service Inspector employee, while working, experienced a headache and became unconscious and later deceased at the hospital. The employer did not report the employee death to the Occupational Safety and Health Administration. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
