29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) 1226 Bankhead Highway #A, Carrollton, GA. 30116, The employer failed to establish a lockout/tagout program which consisted of energy control procedures, employee training and periodic inspections regarding servicing or maintenance on vehicles. An employee was removing a power steering gearbox from a 2001 Chevrolet Tahoe truck, (VIN 1GNEC13T31J177170), supported by two concrete blocks. The employee was crushed between the ground and the A-Frame of the vehicle, resulting in fatal injuries.
