Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a.At the Ocean Golf Course and grounds, employees were exposed to the hazard of personal injury caused by failure to receive proper instruction before operating a Toro Sand Pro machine according to the manufacturers instructions. On October 17, 1994, an employee was fatally injured when the steering chain broke on a Toro Model #08880 Sand Pro #14 Sand Raker machine. The employee was not adequately trained in the maintenance and safety instructions required by the manufacturer in that, employees did not read and understand the contents of the operator's manual such as ensuring that the traction interlock switch is adjusted correctly, checking the interlock system at the beginning of each day, wearing long pants, making sure the neutral return bracket is adjusted and making sure the steering chain and sprocket is inspected and maintained daily. Among other methods, one feasible and acceptable abatement method is to: 1.Adequately train all employees according to the manufacturers operator's manual before and employee is allowed to operate any machine. 2.Clean machine daily, especially make sure the steering chain is free of sand. 3.Lubricate the steering chain when the chain is stiff or rusty; use drytype lubricant and lube lightly; inspect the chain daily. 4.Check operation of interlock system daily before use.
