29 CFR 1910.146(j)(2): The employer did not ensure that each entry supervisor verified, by checking that the appropriate entries had been made on the permit, that all tests specified by the permit had been conducted and that all procedures and equipment specified by the permit had been in-place before endorsing the permit and allowing entry to begin. On or about October 8, 2015, at the New Orleans Facility, the employer failed to ensure that an entry supervisor verified the entry permit for a railcar before entry was allowed. Dedicated TCS, LLC was previously cited for a violation of this Occupational Safety and Health Standard, 29CFR 1910.146(j)(2), which was contained in OSHA inspection number 415682, citation number 1, item number 7 and was affirmed as a final order on November 20, 2012 with respect to a workplace located at 17356 Torrence Ave, Lansing, IL 60438. Dedicated TCS, LLC was previously cited for a violation of this Occupational Safety and Health Standard, 29CFR 1910.146(j)(2), which was contained in OSHA inspection number 987019, citation number 2, item number 5 and was affirmed as a final order on May 18, 2015 with respect to a workplace located at 23330 South Frontage Road W, Channahon, IL 60410.
