Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling onto running conveyors and being crushed by equipment that the conveyors lead to, on or about August 12, 1992. Remotely and automatically controlled conveyors where operator's stations were not manned or were beyond voice and visual contact from drive areas, loading areas, transfer points, and other potentially hazardous locations on the conveyor part were not furnished with emergency stop buttons, pull cords, limit switches, or similar emergency stop controllers, all easily identifiable, in the immediate vicinity of such locations: (a) Saw Mill area - The 300 feet long conveyor was not provided with emergency stop or other acceptable device. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install an emergency stop alongside the conveyor, as required by ANSI B20.1 - 1972, Section 5, 5.15.2 Requirements, 5.15.2.5.3.
