Text For Citation: 01 Item/Group: 009 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an emplosion and fire due to inadequate atmospheric testing for flammable vapors in tanks and vessels: (a) #3 Blow tank/acculator - Prior to cutting or welding testing for flammable vapors, only where the work was being performed, was insufficient to detect the reaining flammable vapors in the tank. On or about February 3, 1993. Among other methods, one feasible and acceptable abatement method correct this hazard is for the employer to develop, implement and maintain an effective written testing procedures for cutting and welding on all tanks that contained flammable/combustible vapors to insure that the total atmosphere of the tank has been tested to determine that all vapors have been removed before allowing work to begin.
