Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) On site of Hobson Road, Frostproof, FL., and other company sites - Employees were erecting an Andrew communications tower on April 10, 1997 and were at the 160 foot level. The employer failed to enforce the use of fall protection, in that, employees were not tied off to the vertical rope lifeline and were not connected to any anchorage point with a lanyard to the "d" ring of the back of their harnesses. The employee wa in motion, securing the tower section, with his harness secured around his waist, (not wearing shoulder straps or thigh straps), lanyard and rope grab along with a rope positioning device were secured back to his belt "d" rings and not being utilized, and fell to the ground, on or about 04/10/97. Among other methods, one feasible and acceptable abatement method to correct this hazard is to enforce and utilize the company issued fall protection equipment, and to make site determinations for the fall hazards of each specific site. IN THE ALTERNATIVE: 29 CFR 1926.105(a): Safety nets were not provided when workplaces were more that 25 feet above the ground or water surface, or other surface(s) where the use of ladders, scaffolds, catch platforms, temporary floors, safety lines, or safety belts was impractical: (a) On site off Hobson Rd., Frostproof, FL. and other company sites - Employees erecting an Andrew communications tower on April 10, 1997 and were at the 160 foot level. The employer failed to enforce the use of personal fall protection systems, in that employees were not tied off to the vertical rope lifeline and were not connected to any anchorage point with a lanyard to the "d" ring of the back of their harnesses. The employee was in motion, securing the tower section, with his harness secured around his waist, (not wearing shoulder straps or thigh straps), lanyard and rope grab along with a rope positioning device were secured back to his belt "d" rings and not being utilized, and fell to the ground, on or about 04/10/97.
