P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of: A- Kiln #3 - The high pressure steam pipe that runs underground from the Boiler room onto the Kiln #3 that was not provided with steam traps to eliminate the condensed steam, allowing this accumulated water to be dragged on by the flow and create a water hammering effect, which cause the pipes and or valves to explode, therefore exposing nearby employees to the hazards of being struck-by flying parts from the pipes and/or valves and from the hazards of skin burns from the escaping steam, on or about 09/02/98. Among other methods, one feasible and acceptable method to correct this hazard is to install steam traps, similar to the steam traps previously installed to the underground steam pipes for the Kiln #1 and Kiln #2, and in operation since 1993 and 1994 respectively. Evaluate the process to determine the effects, if any, of the differences in pipe and valves sizes.
