29 CFR 1926.552(c)(8): Personnel hoist doors or gates were not provided with electrical contacts which do not allow movement of the hoist when door or gate was open: Employees were exposed to fall hazards, shearing or crushing injuries, and trip hazards when operating or riding in the Alimak personnel hoist where the electrical contacts safety device did not prevent the hoist car from moving when the door was in the open position: a) On September 1, 2015, a hoist operator was fatally injured when he fell to the ground 38 feet below the C1 landing when the hoist car moved up from the C1 landing to the C3 landing with the door in the open position. b) On or about August 25, 2015, workers were entering and exiting the left hoist car at the C1 landing, as two workers were pushing a rubbish cart out of the hoist, the car began to ascend with the orange hoist car door in the up position; the hoist car went up approximately two feet before the operator stopped it. c) Two to three weeks prior to the fatal incident, approximately 15 workers were entering and exiting the left hoist car at the C1 landing when the car started to move up with the car door in the up position, the car moved up about 2-3 feet before the operator stopped it.
